C-58 Additive Change Order #2 for Re-Roofing Buildings 1,2,3 & 4 at Clay Hill Elementary (Revised)

Issue/Rationale
Change orders are initiated by the Contractor, Architect/Engineer or Owner, which may increase or decrease the scope of the project as defined by the plans and specificiations.  Once a change order is initiated, review/evaluation is undertaken to determine the merit and/or requirements of the change order.  Change orders affect the scope of the project and are required to be School Board approved by State Requirements for Educational Facilities and Florida Statutes.  An additional twenty-two (22) days for Buildings 1 & 2 (Media & Cafeteria) and twenty-nine (29) days for Buildings 3 & 4 (Boiler & Storage) are required due to the change in the scope of work (See attached Change Order #2).  This change will move that date to on or before, August 4, 2006 for Buildings 1 & 2 and on or before August 11, 2006 for Buildings 3 &4.  Final completion will also be adjusted 15 days after substantial completion (as per project specifications) or to August 19, 2006 and August 26, 2006 respectively.  This Change Order is absolutely necessary.  The plywood roof deck, due to poor ventilation, is deteriorated so badly from extreme heat and age that failure is eminent.  This condition was not known when the re-shingle project was originally bid and was only discovered when the old shingles were removed.  Time is of extreme importance as this project has now become considerably more extensive.

Recommended Action
Approve Change Order #2 as submitted.

Financial Impact
This Change Order will increase the project cost by $126,200.00 which will initially be funded by Fund Balance as no 2005-2006 Special Maintenance funding remain available.  The Fund Balance will be re-paid with 2006-2007 Special Maintenance Funds once received.
